Can chronic osteomyelitis turn into bone cancer?
Chronic osteomyelitis rarely transforms into bone cancer, as the underlying pathogenesis of these two conditions is fundamentally distinct. Malignant transformation occurs only in an extremely small number of cases—typically those with exceptionally prolonged disease duration and repeated inflammatory stimulation—and the overall probability of such transformation is exceedingly low. Persistent non-healing sinus tracts and progressively worsening pain warrant prompt medical evaluation to rule out malignancy.

Chronic osteomyelitis is a persistent inflammatory condition of bone tissue caused by bacterial infection, most commonly evolving from inadequately treated acute osteomyelitis. The disease course is protracted and recurrent, often manifesting as bone destruction, sequestrum (dead bone) formation, and draining sinuses. The core pathology involves an inflammatory response triggered by bacterial infection; bone cells themselves do not exhibit abnormal proliferation or malignant potential. Consequently, in the majority of patients, the disease remains confined to the inflammatory stage and does not progress to bone cancer.
The primary treatment objective for chronic osteomyelitis is eradication of the infectious focus. This typically entails surgical debridement—including removal of necrotic bone, sinus tract excision, and adequate drainage of purulent material—followed by a full course of appropriate antimicrobial therapy to control infection. With such comprehensive management, most patients achieve stable disease control. Malignant transformation, though theoretically possible, occurs only after decades of chronic inflammation with repeated local tissue irritation—and even then, the risk remains extremely low. If malignancy is confirmed, wide surgical resection of the affected tissue is required to halt disease progression.
In daily life, patients should protect the affected limb against trauma or impact injury, maintain a balanced diet rich in protein and vitamins to bolster immune function, and keep the skin surrounding any sinus tract clean and dry to minimize the risk of acute inflammatory exacerbations and preserve bone health.
