Back pain upon waking may result from improper sleeping posture, an unsuitable mattress (either too soft or too firm), lumbar or thoracic myofascial inflammation, thoracic facet joint dysfunction, or back muscle strain. It can be improved by adjusting sleeping posture, replacing the mattress with one of appropriate firmness, and using targeted medications. If morning back pain persists without relief, prompt medical evaluation is recommended.

3. Lumbar–thoracic myofascial inflammation: Chronic irritation of the back fascia leads to sterile inflammation, accompanied by fascial hyperemia and edema. Inflammatory stimulation intensifies during nighttime immobility, resulting in stiffness and soreness upon waking. Under physician guidance, topical flurbiprofen gel patches, sustained-release ibuprofen capsules, or eperisone hydrochloride tablets may be used to alleviate symptoms.

4. Thoracic facet joint dysfunction: Minor misalignment of the thoracic facet joints stretches surrounding ligaments and soft tissues. Nighttime sleeping positions alter joint loading, leading to back pain upon waking—often worsening with movement or twisting. Under medical supervision, loxoprofen sodium tablets, celecoxib capsules, or compound chlorzoxazone tablets may be prescribed to relieve discomfort.

5. Back muscle strain: Repetitive heavy loading or overuse causes chronic microtrauma to the back muscles. During nighttime rest, reduced circulation slows metabolic waste clearance, leading to accumulation and subsequent back pain upon waking. As directed by a physician, diclofenac sodium gel, etoricoxib tablets, or mecobalamin tablets may be used to improve symptoms.

In daily life, maintain proper posture, avoid prolonged sitting or standing, choose a supportive mattress, keep the back warm, and perform gentle stretching exercises for the lumbar and thoracic musculature to relieve soft-tissue tension.
