When is kidney transplantation indicated?
Kidney transplantation is indicated for patients with end-stage renal disease (ESRD), chronic kidney failure, inadequate response to medical therapy, suboptimal dialysis outcomes, and overall good surgical tolerance. Severe renal dysfunction impairs the body’s ability to maintain normal metabolism. If symptoms such as severe anemia, persistent edema, or life-threatening electrolyte imbalances occur, prompt medical evaluation and treatment are essential.
1. End-Stage Renal Disease (ESRD): Widespread destruction of renal tissue results in near-complete loss of glomerular filtration and excretory function, rendering the body incapable of autonomously eliminating metabolic waste. Accumulation of toxins leads to progressive damage across multiple organ systems; conventional conservative management can no longer sustain normal physiological function.
2. Chronic Kidney Failure: Progressive deterioration of renal function occurs secondary to various underlying kidney diseases, resulting in irreversible decline. As the kidneys’ regulatory and detoxification capacities progressively diminish, multisystem symptoms emerge and worsen over time, necessitating renal replacement therapy to sustain life.

3. Inadequate Response to Medical Therapy: Despite standardized pharmacological management, renal function continues to deteriorate. Pharmacotherapy fails to halt further progression of kidney failure, and multiple complications increasingly develop—making continued reliance solely on medication inappropriate.
4. Suboptimal Dialysis Outcomes: Despite prolonged hemodialysis or peritoneal dialysis, clinical status remains unsatisfactory. Recurrent severe complications and progressive decline in quality of life indicate that dialysis no longer adequately meets metabolic demands—warranting evaluation for kidney transplantation.
5. General Surgical Tolerance: Key organ systems—including the heart, lungs, and liver—function well; there is no active, uncontrolled infection or malignancy. The patient is physiologically capable of tolerating both the transplant surgery itself and subsequent immunosuppressive therapy, fulfilling the fundamental prerequisites for transplantation.
Postoperatively, maintain a light, nutritionally balanced diet with controlled sodium and protein intake. Adhere strictly to prescribed medications, implement daily infection-prevention measures, and undergo regular monitoring of renal function to ensure long-term stability and optimal function of the transplanted kidney.
