Under what circumstances is wisdom tooth extraction necessary?
Indications for wisdom tooth extraction include recurrent pericoronitis, dental caries, impingement on adjacent teeth or tissues, abnormal eruption position, and cystic or other pathological lesions.
If extensive swelling of the jaw and face or severe trismus (limited mouth opening) occurs, prompt medical evaluation and treatment at a hospital are recommended.
1. Recurrent pericoronitis: Partially erupted wisdom teeth often trap food debris beneath the overlying gingival flap, fostering bacterial growth and triggering pericoronitis. Symptoms include gingival swelling and pain; inflammation may recur and spread to involve the pharynx and parotid region, interfering with normal eating.
2. Dental caries: Due to their posterior location, wisdom teeth are difficult to clean thoroughly during routine oral hygiene, making them prone to decay. Caries can progress progressively into deeper dental structures, causing pain. Restorative treatment is often technically challenging due to poor access and unfavorable positioning; therefore, extraction is generally recommended.

3. Impingement on adjacent teeth/tissues: When wisdom teeth erupt at an abnormal angle, they may continuously press against the adjacent healthy molars. Prolonged mechanical pressure can lead to root resorption of the adjacent tooth, alveolar bone destruction, and accumulation of dental plaque in interdental spaces—increasing the risk of caries and periodontal damage in neighboring teeth.
4. Abnormal eruption position: Wisdom teeth may erupt in various malpositioned orientations—including mesioangular (tilted forward), horizontal, or inverted—and lack sufficient space to establish functional occlusion. Such teeth do not contribute to mastication and may disrupt dental arch alignment, increasing the likelihood of occlusal disturbances.
5. Cystic or other pathological lesions: A dentigerous cyst or similar cystic lesion may develop around the unerupted wisdom tooth germ. As it gradually expands, it erodes and destroys surrounding jawbone. Early-stage lesions are typically asymptomatic; however, as the lesion enlarges, progressive bone resorption occurs, potentially compressing adjacent nerves and teeth, resulting in neurological symptoms or structural damage.
After wisdom tooth extraction, patients should monitor food temperature and texture, maintain meticulous oral hygiene, and observe for signs such as bleeding, swelling, or pain at the surgical site. Avoid repeatedly touching the wound with the tongue, and minimize consumption of spicy or hard foods to facilitate smooth and uncomplicated healing of the extraction socket.
