  Rupture of membranes (ROM), commonly known as “water breaking,” typically presents as involuntary, uncontrolled leakage of clear fluid from the vagina. The volume may vary—ranging from minimal to substantial—and is unaffected by body position or abdominal pressure. This distinguishes it clearly from urinary incontinence or increased vaginal discharge. Any persistent, unexplained vaginal fluid leakage during pregnancy warrants prompt medical evaluation and management.

  Following rupture of the fetal membranes, amniotic fluid leaks out. In cases of high-lying (or “high”) membrane rupture, the volume of fluid may be small and intermittent—often only a slight seepage—making it easily mistaken for normal vaginal discharge. In contrast, low-lying (or “low”) membrane rupture usually results in a sudden gush of fluid, with increased leakage upon standing, walking, or straining (e.g., during coughing). During late pregnancy, when the nature of the fluid is uncertain, specialized clinical assessment—including speculum examination, pH testing, fern test, or ultrasound—can help confirm membrane status. Once ROM is diagnosed, subsequent management must be tailored according to gestational age.

  Prolonged leakage of amniotic fluid reduces the protective cushioning around the fetus and increases the risk of intrauterine infection. For term pregnancies (≥37 weeks) with ROM, labor is generally induced promptly. In preterm pregnancies (<37 weeks) complicated by preterm premature rupture of membranes (PPROM), management decisions—including expectant management (with close monitoring and possible tocolysis or corticosteroids) versus delivery—depend on gestational age, maternal and fetal condition, and markers of infection. Continuous fetal heart rate monitoring and maternal temperature surveillance are essential throughout to mitigate risks to both mother and fetus.

  To minimize further fluid loss, lie supine with hips slightly elevated (e.g., using a pillow under the buttocks). Use clean, breathable sanitary pads (avoid tampons), limit ambulation, and maintain meticulous perineal hygiene to reduce the risk of ascending infection. Remain calm and stable while awaiting definitive clinical management.
