Introduction
Red spots appearing on both shoulders and the back are not necessarily due to an allergic reaction. Such rashes have multiple possible causes; allergic dermatitis is only one of them—folliculitis and Malassezia infection can also produce similar skin lesions. Differentiation requires careful evaluation of accompanying symptoms. Prompt medical consultation is essential if the red spots continue to increase in number or are accompanied by pain, worsening pruritus (itching), or other concerning symptoms.

Red spots appearing on both shoulders and the back are not necessarily due to allergies. Such rashes have multiple possible causes; allergic dermatitis is only one of them—folliculitis and Malassezia infection can also produce similar skin lesions. Differentiation requires careful evaluation of accompanying symptoms. Prompt medical consultation is advised if the red spots continue to increase in number or are accompanied by worsening pain or pruritus.

Rashes caused by allergies are typically associated with marked pruritus and often appear after exposure to new clothing or personal care products. The rash borders may be indistinct, and pustules are generally absent. Management primarily involves eliminating exposure to the allergen. Topical soothing and antipruritic agents may be applied; for severe pruritus, oral antihistamines are recommended. Scratching should be minimized to prevent skin trauma and secondary infection.

The back contains numerous hair follicles, and excessive sweating combined with sebum accumulation predisposes individuals to folliculitis. Affected lesions commonly feature white pustules at their apex and are tender upon palpation. Bacterial folliculitis may be treated with topical antibacterial ointments, whereas fungal folliculitis requires antifungal agents. Avoiding hot, humid environments and refraining from squeezing lesions help prevent inflammatory spread and post-inflammatory hyperpigmentation.

Wear loose-fitting, breathable cotton clothing; rinse the shoulder and back areas promptly after sweating to maintain local dryness and cleanliness. Reduce intake of high-sugar and high-fat foods, and regularly change pillowcases and bed linens to lower the risk of recurrent rashes.
