Is thoracotomy always required for mediastinal tumor surgery?
Thoracotomy is not the only option for treating mediastinal tumors—especially today, as technological advances have enabled an increasing number of minimally invasive procedures for their management. These include muscle-sparing approaches with small incisions, the most commonly performed video-assisted thoracoscopic surgery (VATS), and, increasingly, subxiphoid mediastinal tumor resection, now offered at more and more thoracic surgical centers.
Additionally, robotic-assisted mediastinal tumor resection using the da Vinci Surgical System is also available. However, all the aforementioned minimally invasive techniques serve merely as adjuncts to conventional thoracotomy—not as complete replacements. For tumors located in anatomically challenging positions or those of large size, lateral or median sternotomy remains a reasonable and appropriate surgical approach.
