Introduction
What causes white patches on a 2-year-old’s scalp? It is relatively uncommon for a 2-year-old to develop vitiligo, but it remains a possibility. Another potential cause is pityriasis alba—also known as “white pityriasis.” This condition is more likely, as pityriasis alba predominantly affects infants and young children and commonly occurs on the face and scalp.

For a 2-year-old child presenting with white patches on the scalp, it is important to assess the degree and appearance of the whiteness. If the patches are porcelain-white—resembling the color of a wall or ceramic—they may indicate vitiligo. Although vitiligo is relatively uncommon in 2-year-olds, it remains a possibility. Alternatively, the condition could be pityriasis alba (also known as “white pityriasis”), which is comparatively more common in this age group. Pityriasis alba predominantly affects infants and young children and frequently occurs on the face and scalp. Its development is often associated with localized impaired microcirculation, gastrointestinal dysfunction, or sun exposure.
