The patient will not experience significant pain. Prostate biopsy is the gold standard for precise diagnosis of prostate diseases—especially prostate cancer—as a definitive diagnosis cannot be made without biopsy. However, many patients fear prostate biopsy. In fact, it is a minimally invasive diagnostic procedure that is relatively simple to perform, associated with minimal trauma, and causes very little discomfort.

Prostate biopsy is a minimally invasive procedure in which a biopsy needle is used to obtain prostate tissue samples. It is the most definitive diagnostic method for confirming prostate cancer—the so-called “gold standard” in medicine. With advances in diagnostic technology and the use of extremely fine biopsy needles, prostate biopsy has become a very safe procedure. So, is prostate biopsy painful? Patients typically do not experience significant pain. Prostate biopsy remains the gold standard for precise diagnosis of prostate disease—especially for prostate cancer—as definitive diagnosis cannot be made without biopsy. Nevertheless, many patients harbor apprehension about undergoing prostate biopsy. In reality, however, it is a minimally invasive diagnostic procedure that is relatively simple to perform, associated with minimal trauma and little discomfort. Biopsy is primarily performed under imaging guidance—most commonly transrectal ultrasound (TRUS). During the procedure, local anesthesia or spinal (epidural) anesthesia may be administered to effectively block pain sensation. Consequently, patients generally do not experience noticeable pain or discomfort during the biopsy.

Knowledge Extension: Techniques for Prostate Biopsy

1. Transrectal biopsy under local anesthesia: Only topical anesthetic is applied to the rectal mucosa prior to biopsy. Since a fine needle is used, tissue injury is minimal and discomfort is low.

2. Transperineal biopsy: Requires spinal or intravenous sedation/anesthesia before needle insertion through the perineum to obtain tissue specimens for definitive diagnosis of prostate cancer. Discomfort associated with this approach is also minimal.
