Can apical periodontitis heal spontaneously?
Apical periodontitis is a relatively common condition in modern society, typically resulting from inadequate dental hygiene that allows bacteria to proliferate and invade the tooth root.
Can apical periodontitis resolve spontaneously?
Apical periodontitis of the tooth root cannot resolve spontaneously. It can be classified into acute apical periodontitis—such as acute serous apical periodontitis. If pain becomes unbearable, temporary relief may be achieved with anti-inflammatory and analgesic medications administered topically, orally, or via injection. However, once pain subsides, prompt medical evaluation and treatment are essential.

If acute serous apical periodontitis remains untreated, it may progress to acute suppurative apical periodontitis. Left unmanaged, acute suppurative apical periodontitis can further evolve into chronic apical periodontitis—manifesting as chronic apical abscess, chronic apical cyst, or chronic apical granuloma. Although chronic apical periodontitis is generally less painful than its acute counterpart, this does not indicate spontaneous resolution; rather, it signifies progression to a chronic stage.
