Introduction
Once hemolysis occurs, jaundice is the most prominent and earliest symptom. As hemolysis persists, patients may develop anemia of varying severity. Hepatosplenomegaly may also occur. In severe cases, patients may exhibit symptoms of hemolytic encephalopathy, such as lethargy and fixed gaze.

Hemolytic disease refers to a condition caused by blood type incompatibility between a pregnant mother and her fetus. Clinically, it primarily comprises two major types: ABO hemolytic disease and Rh hemolytic disease. ABO hemolytic disease most commonly occurs when the mother has type O blood and the infant has either type A or type B blood. What are the earliest signs of maternal-fetal hemolysis? Once hemolysis occurs, jaundice is the most prominent and earliest symptom. As hemolysis persists, patients may develop varying degrees of anemia. Hepatosplenomegaly may also occur. In severe cases, symptoms of hemolytic encephalopathy—such as lethargy and fixed gaze—may appear.

Phototherapy is generally effective in alleviating jaundice in affected infants. Symptomatic treatment is also critical: severely anemic patients may receive transfusions of washed red blood cells. Plasma or albumin infusions may be administered to reduce the risk of hemolytic encephalopathy. Exchange transfusion is another effective therapeutic option, and glucocorticoids are frequently used medications.

Dietary recommendations include consuming iron-rich foods such as black fungus, jujubes (red dates), black rice, spinach, lean meat, milk, and eggs. Foods that commonly trigger allergic reactions—such as fish, shrimp, and crab—should be minimized. For patients with a bleeding tendency, a residue-free semi-liquid diet is advised; foods containing bones or sharp spines should be avoided to prevent injury-related bleeding and infection. Fresh vegetables and fruits are encouraged.
