Introduction
First, genetics: Genetic factors are considered the primary cause of vulvar lipomas, as these tumors exhibit a marked familial clustering tendency. Second, liver function: There is an association between liver function and vulvar lipomas, as most patients with vulvar lipomas have impaired liver function. Third, emotional factors: Irritability and anxiety can significantly exacerbate vulvar lipoma symptoms.

Vulvar lipomas arise in the mons pubis, labia, or other areas of the vulva containing adipose tissue. These tumors vary in size, most commonly presenting as small pedunculated or sessile polypoid lesions; occasionally, they may reach a diameter of 10–12 cm. They are typically soft in consistency, exhibit a lobulated surface, and are encapsulated. Vulvar lipomas grow slowly and rarely undergo malignant transformation. So, what causes vulvar lipomas? The following section addresses this question.

What Causes Vulvar Lipomas?

First, genetics: Genetic factors are considered the primary etiological factor for vulvar lipomas. A strong familial clustering—especially in cases of multiple vulvar lipomas—is frequently observed. Infectious agents such as bacteria, viruses, or parasites can be readily ruled out as causative factors; instead, there is a well-established association between vulvar lipomas and genetic predisposition.

Second, liver function: Hepatic dysfunction is associated with vulvar lipomas. Most patients with vulvar lipomas exhibit impaired liver function and have a history of heavy alcohol consumption. Postoperative prognosis—including recurrence rates—demonstrates that continued alcohol intake correlates with poorer outcomes, whereas abstinence from alcohol significantly improves prognosis and reduces recurrence risk. This relationship likely stems from the liver’s central role in lipid synthesis and metabolism.

Third, emotional factors: Chronic stress, irritability, and anxiety can markedly exacerbate vulvar lipoma progression; therefore, maintaining an optimistic and relaxed mental state is advisable. Obesity is another significant contributing factor: Individuals with obesity have a substantially higher incidence of vulvar lipomas compared to those with normal body weight.Other potential contributing factors include renal dysfunction.
