1. Psychological factors: Lack of adequate sexual knowledge, excessive preoccupation with sexual matters, and heightened susceptibility to sexual stimuli can lead to persistent cortical excitation, thereby triggering nocturnal emission.

2. Environmental sexual stimulation: Exposure to sexually suggestive material—such as erotic literature or films—can stimulate the brain and precipitate nocturnal emission.

3. Excessive fatigue: Prolonged physical or mental exertion leads to bodily exhaustion and enhances subcortical central nervous system activity during sleep, predisposing to nocturnal emission.

4. Inflammatory stimuli: Inflammation of the external genitalia or accessory sex glands—including balanoposthitis, prostatitis, seminal vesiculitis, and epididymitis—may trigger nocturnal emission.

5. Physical factors: Sleeping supine under heavy, warm bedding—which may compress or irritate the external genitalia—or wearing tight-fitting underwear that constricts or applies pressure to an erect penis can induce nocturnal emission.

6. “Overflow due to fullness”: The testes continuously produce sperm, while the seminal vesicles and prostate gland continually secrete seminal fluid. When seminal volume reaches a certain threshold, semen may spontaneously discharge via the urethra.

Treatment of nocturnal emission is critically important. Patients should cultivate healthy lifestyle and daily routines, maintain emotional well-being, and engage in appropriate outdoor physical exercise. During active episodes, they should eliminate distracting thoughts, moderate sexual desire, and strictly avoid frequent masturbation. When necessary, soaking the feet in warm water before bedtime—and gently massaging the soles—may be beneficial. Additionally, adopting a lateral sleeping position is recommended.
