Smegma is a substance formed by the chemical reaction between sebum secreted by the sebaceous glands of the prepuce and urine. In cases of phimosis or redundant prepuce, the foreskin cannot be retracted, causing this sebum to accumulate in the space between the inner surface of the foreskin and the penis. Smegma is a chemical carcinogen; therefore, maintaining good personal hygiene and regularly cleaning the foreskin is essential.

The accumulation of smegma can adversely affect male genital health, causing local itching and pain, and potentially interfering with sexual function. Why does smegma form?

Why Does Smegma Form?

Smegma is formed through a chemical reaction between sebum secreted by the sebaceous glands beneath the foreskin and urine residues. In cases of phimosis or redundant (excessively long) foreskin, the foreskin cannot be fully retracted, allowing sebum to accumulate in the space between the inner surface of the foreskin and the glans penis. Smegma contains carcinogenic chemical substances; therefore, maintaining good personal hygiene—including regular cleaning of the foreskin—is essential. Individuals with phimosis or significantly redundant foreskin should seek timely medical evaluation and treatment.

Smegma formation commonly occurs due to phimosis—characterized by a narrow preputial opening—or inability to retract the foreskin sufficiently to expose the urethral meatus or glans penis. In cases of redundant foreskin, failure to regularly retract and clean beneath the foreskin contributes to smegma accumulation. This condition is relatively common. Typically, mild adhesions between the glans and foreskin present after age three gradually resolve spontaneously, allowing for full retraction.

If you have a foreskin, it is advisable to maintain meticulous local hygiene—washing the area daily with warm water. Additionally, keep the area dry and clean, and change undergarments daily. Most cases of smegma result from redundant foreskin.
