Traditional Chinese medicines (TCMs) for clearing “fire” are categorized into two types: those for clearing excess fire and those for clearing deficient fire. TCMs for clearing excess fire mainly include Qingwei Huanglian Pills, Niuhuang Qingwei Pills, Niuhuang Qingfei Pills, Qinkailing Capsules, Shuanghuanglian Capsules, Yankening Tablets, and Huanglian Shangqing Tablets. These formulas tend to be cold in nature and should therefore be used with caution—ideally only under the guidance of a qualified TCM practitioner. Once symptoms improve, treatment should be discontinued promptly; prolonged use may damage the body’s yang qi, potentially leading to symptoms such as gastric coldness and diarrhea. Furthermore, accurate pattern differentiation is essential to identify the specific affected organ systems and select targeted treatments accordingly. “Excess heat” and “deficiency heat” manifest differently. Common symptoms of excess heat include red, swollen, and painful eyes; dry mouth and throat; sore tongue; sore throat; swollen and painful gums; epistaxis (nosebleeds); tinnitus or hearing loss; painful sores or abscesses; irritability; scanty, dark urine; constipation; a red tongue with yellow coating; and a rapid, thin pulse.
