Calf muscle pain after physical education class may result from overexertion, muscle strain, or other causes. Specific reasons include the following:

1. Overexertion: Calf muscle pain following physical education class may occur due to sudden, intense physical activity—especially if one is habitually sedentary and then engages abruptly in strenuous exercise such as hiking. This often leads to calf muscle soreness, which typically improves with rest. Accumulation of lactic acid in the muscles outpaces its metabolic clearance, resulting in muscle soreness and pain. If the pain is transient, resolves with rest, and recurs upon resuming activity, it is most likely attributable to excessive exertion.

2. Muscle Strain: Inadequate warm-up before exercise or excessive physical activity can exceed the physiological limits of the calf muscles, leading to gastrocnemius strain—and even minor intramuscular hemorrhage—causing localized pain. Severe, progressively worsening calf pain post-exercise that precludes continued activity usually indicates injury to the gastrocnemius tendon, or possibly intramuscular bleeding secondary to overuse. In such cases, patients should undergo ultrasound or MRI of the calf to rule out tendon strain or partial tendon tear.
