Diseases of the biliary or hepatic systems: When disease affects the liver or biliary system, serum bilirubin levels increase. Elevated bilirubin subsequently alters the color of the sclera, resulting in scleral icterus (yellowing). This etiology occurs not only in adults but also frequently in newborns. Therefore, in routine clinical practice, scleral yellowing should prompt consideration of hepatic or biliary disease.

Common causes of yellowing eyes include:
Diseases of the biliary system or liver: When the liver or biliary system is diseased, blood bilirubin levels rise. Elevated bilirubin affects the color of the sclera, causing scleral icterus (yellow discoloration). This condition occurs not only in adults but also frequently in newborns. Therefore, in routine clinical practice, scleral yellowing should prompt consideration of hepatic or biliary disease.

What Causes Mild Yellowing of the Sclera?

Patients are advised to consult relevant departments—such as gastroenterology or hepatobiliary surgery—for evaluation to confirm or rule out underlying disease. Only targeted treatment can resolve scleral yellowing.
Physiological scleral yellowing: The sclera is chronically exposed to the external environment, making it susceptible to environmental factors such as sunlight and windblown dust, which cause cumulative surface irritation over time.

Additionally, with advancing age, metabolic activity declines and lymphatic drainage becomes less efficient, leading to delayed clearance of metabolic waste products. These substances may accumulate beneath the conjunctiva or in the scleral tissue, resulting in yellow discoloration. Currently, no definitive treatment exists for this physiological yellowing. However, warm compresses applied to the eyes and gentle periorbital massage may help improve local blood circulation and metabolism, thereby facilitating the timely removal of metabolic waste and alleviating scleral yellowing.
