The earliest sign of diminished sexual function is often impaired penile erection. Erections may lack sufficient rigidity; even if hardness is marginally adequate for intercourse, maintaining the erection until ejaculation proves difficult—resulting in detumescence (loss of erection) mid-coitus. Consequently, the female partner’s sexual needs remain unmet, and adequate sexual stimulation cannot be provided. Diminished sexual function also includes shortened duration of intercourse: some men ejaculate within three minutes—or even upon initial vaginal contact—indicating clinically significant premature ejaculation. Both erectile dysfunction (ED) and premature ejaculation constitute sexual dysfunctions that can profoundly impact a man’s self-esteem.
