Introduction
Methods to facilitate conception in women with a retroverted uterus include the following: 1. Elevating the buttocks after intercourse—placing a pillow under the woman’s buttocks during or after intercourse. 2. Adopting a prone position (lying face down) after intercourse, or assuming a rear-entry (doggy-style) position during intercourse. 3. Surgical interventions such as round ligament shortening or uterine suspension. 4. Use of a pessary.

During routine health examinations, some women discover they have a retroverted uterus (a uterus tilted backward). A major concern associated with a retroverted uterus is reduced fertility. So how can conception be facilitated? The following methods may help:

① Elevate the buttocks after intercourse

Place a pillow under the woman’s buttocks during or immediately after intercourse to position her in a head-low, hip-high posture. This allows semen to pool in the posterior vaginal fornix, immersing the cervix in the semen reservoir and thereby enhancing the chances of conception.

② Adopt the “rear-entry” (doggy-style) position

This refers specifically to either assuming a prone position after intercourse or adopting a rear-entry position during intercourse. Such positioning encourages semen accumulation in the anterior vaginal fornix, facilitating cervical immersion in semen—particularly beneficial when the cervix is tilted upward.

③ Round ligament shortening or uterine suspension surgery

It should be emphasized that the efficacy of these surgical interventions remains uncertain, and recurrence is common within several years. Consequently, these procedures are currently rarely recommended.

④ Uterine pessary correction

In select cases, manual repositioning of the uterus into an anteverted (forward-tilted) position may first be performed, followed by short-term placement of a uterine pessary to achieve temporary correction.
