Cold urticaria is a subtype of urticaria whose primary clinical manifestations include pruritus and wheals developing on various body parts after exposure to cold conditions. Skin lesions are confined to areas exposed to cold or in direct contact with cold objects; oral mucosal involvement may also occur. Systemic symptoms—such as headache, chills, diarrhea, and tachycardia—can accompany the cutaneous manifestations. Because opportunities for exposure to cold objects are frequent during winter, this condition is relatively common.

There are no highly specific medications for treating cold urticaria. The mainstay of management involves avoiding cold stimuli—particularly damp cold—and minimizing rapid changes in skin temperature. For example, during summer, when ambient temperatures are high, one should acclimatize gradually upon entering an air-conditioned room and take care to stay warm. As pharmacologic options lack strong specificity, oral antihistamines—including conventional agents like cetirizine and newer-generation drugs such as levocetirizine and loratadine—may be used.

In cases of severe cold urticaria—especially with prominent systemic symptoms—short-term corticosteroid therapy may be warranted. Another therapeutic approach is cold desensitization, which involves controlled exposure to cold water (e.g., cold-water bathing); however, this carries certain risks and must be conducted under medical supervision. Oral medications alone do not constitute a specific curative treatment.
