Surgical correction of O-shaped legs is typically performed under combined spinal-epidural anesthesia or general anesthesia. A longitudinal, arc-shaped incision approximately 8 cm in length is made along the lateral aspect of the affected leg. The superficial and deep fasciae are then incised to expose the inferior and lateral aspects of the tibial plateau. Based on preoperative planning, a wedge osteotomy is performed at the predetermined angle. Following osteotomy, the distal and proximal segments of the tibia are realigned and securely fixed using an osteotomy plate. Intraoperative fluoroscopic imaging (C-arm) is used to verify the adequacy of correction. Once satisfactory alignment is confirmed, the deep fascia, superficial fascia, and skin are closed layer by layer.
