What Causes an Unusually White and Thick Tongue Coating?
A white and thick tongue coating primarily results from two factors: (1) chronically poor oral hygiene, and (2) internal “heat” or “fire” (a traditional Chinese medicine concept referring to inflammatory or hyperactive physiological states). When individuals neglect thorough tooth brushing and mouth rinsing over extended periods, food debris and bacteria accumulate on the tongue and teeth surfaces, leading to a whitish, thickened coating and halitosis (bad breath). Such individuals are also more prone to gingivitis and periodontitis, which may manifest as red, swollen, and bleeding gums.

When internal “heat” arises—particularly excessive stomach fire—it may also produce a thick, white tongue coating accompanied by halitosis. This pattern is commonly observed in individuals with hyperactive stomach fire, often resulting from prolonged consumption of greasy, easily digestible foods. Such dietary habits impair digestion, causing food to stagnate in the stomach for extended periods and thereby generating excess stomach fire. Additionally, individuals with irritable temperaments—especially those experiencing chronic anxiety, tension, low mood, depression, or frequent anger—may similarly present with a thick, white tongue coating.

Treatment for a thick, white tongue coating should be tailored to its underlying cause. If inadequate oral hygiene has led to conditions such as glossitis or other oral inflammations, anti-inflammatory medications combined with improved oral hygiene practices are recommended.
