Introduction
Diffuse goiter does not resolve spontaneously without treatment. Numerous factors can cause diffuse thyroid enlargement in patients. For instance, some patients develop endemic goiter due to iodine deficiency; if left untreated, their thyroid enlargement will progressively worsen. Others may have hyperthyroidism or hypothyroidism—both of which can also lead to thyroid enlargement.

Diffuse thyroid enlargement significantly disrupts daily life. Can diffuse thyroid enlargement resolve spontaneously?

Can diffuse thyroid enlargement resolve spontaneously?

Without treatment, diffuse thyroid goiter does not resolve spontaneously. Numerous underlying causes may lead to diffuse thyroid enlargement. For instance, iodine deficiency can cause endemic goiter; without timely and appropriate intervention, the goiter progressively worsens. In other cases, hyperthyroidism or hypothyroidism may be responsible for thyroid enlargement. Treatment must be tailored to the specific etiology—e.g., antithyroid medications—and with appropriate therapy, the goiter typically gradually decreases in size. Conversely, without treatment, the thyroid gland usually continues to enlarge.

Diffuse thyroid enlargement is generally not curable. Most cases stem from autoimmune thyroid disease, the most common being chronic lymphocytic thyroiditis (Hashimoto’s thyroiditis). Patients often test positive for autoantibodies—including anti-thyroid peroxidase (TPO) antibodies and anti-thyroglobulin antibodies—which drive diffuse thyroid enlargement.
