Tip-of-the-tongue “fire,” accompanied by painful oral ulcers on the tongue tip, is commonly diagnosed in Traditional Chinese Medicine (TCM) as an excess of Heart Fire. According to TCM theory, the tongue is considered the “sprout” or “reflection” of the Heart; thus, burning pain at the tongue tip primarily reflects Heat accumulation in the Heart and is treated mainly by clearing Heart Fire and draining Heat. Common therapeutic approaches include ensuring adequate sleep and avoiding staying up late. Sleep deprivation directly and significantly depletes Heart Blood; therefore, chronic sleep deprivation often leads to insufficient Heart Blood, resulting in ascending Fire.

Tongue pain is often one of the manifestations of “excess internal heat” (a traditional Chinese medicine concept). Tongue pain associated with “heatiness” typically results from deficiencies in trace elements, irregular lifestyle habits, and consumption of overly salty, spicy, or greasy foods. What to Do for Tongue Pain Caused by “Excess Heat”

Painful redness or ulcers at the tip of the tongue are commonly attributed in Traditional Chinese Medicine (TCM) to excessive “Heart Fire.” According to TCM theory, the tongue is considered a “sprout” or reflection of the Heart; thus, burning pain at the tongue tip reflects an overabundance of Heart Fire. Treatment focuses primarily on clearing Heart Fire and draining heat. Key recommendations include ensuring adequate sleep and avoiding late-night activity. Staying up late directly depletes Heart Yin and Blood—making it a major contributing factor to Heart Fire. For tongue-tip heat, herbal remedies or food-grade herbs that clear Heart Fire and calm the Shen (spirit) may be used. Chrysanthemum flowers and cassia seeds (Cassia obtusifolia), for example, are highly effective for clearing Heart Fire. Eating fruits such as bananas and dragon fruit, or drinking celery juice, may help regulate bowel function.

For tongue pain caused by “excess heat,” anti-inflammatory medications may be taken orally as prescribed by a physician. Such tongue pain is closely linked to poor dietary habits—especially frequent consumption of dry, spicy, or otherwise irritating foods. During treatment, maintain good oral hygiene: rinsing the mouth with warm salt water helps reduce inflammation and kill bacteria.
