How to Manage Phimosis

1. When phimosis is present, meticulous cleaning is essential. Gently retract the foreskin to fully expose the glans penis—this facilitates thorough cleansing and may also reduce glans sensitivity, potentially helping to prolong intercourse duration.

2. Circumcision is recommended promptly for individuals with phimosis, a narrow preputial opening, recurrent balanoposthitis, or true phimosis (inability to retract the foreskin).

With phimosis, smegma readily accumulates beneath the foreskin, fostering pathogenic bacterial growth and triggering balanoposthitis. Chronic, recurrent balanoposthitis is a well-established high-risk factor for penile cancer. Prolonged coverage of the glans by the foreskin increases its sensitivity, predisposing patients to premature ejaculation. Additionally, a tight foreskin may become trapped behind the coronal sulcus during intercourse—a condition known as paraphimosis—which impedes venous return from both the foreskin and glans, resulting in edema; in severe cases, it may even lead to glans necrosis.

Patients with phimosis are prone to smegma accumulation. Therefore, daily external genital cleansing is advised to maintain cleanliness and dryness of the genital area.
