What Is Thromboangiitis Obliterans?
Thromboangiitis obliterans (TAO), also known as Buerger’s disease, is a rare, chronic, and recurrent inflammatory disorder affecting small- and medium-sized arteries and veins. In the lower limbs, it commonly manifests as ischemic pain, intermittent claudication, and diminished or absent dorsalis pedis artery pulsation. Other characteristic features include migratory superficial phlebitis, and in severe cases, digital ulcers and gangrene. The disease is generally believed to result from vasospasm of small arteries followed by thrombotic occlusion, leading to localized tissue ischemia. Approximately half of affected patients exhibit Raynaud’s phenomenon. TAO predominantly affects males, especially those who smoke.

Smoking is strongly associated with both the development and prognosis of this disease. Clinically, patients typically experience transient or persistent pallor and dryness of the extremities, accompanied by burning or stabbing pain. The skin becomes erythematous when the legs are dependent and blanches upon elevation. Calf muscle pain and numbness may occur, worsening during ambulation and resulting in intermittent claudication—symptoms that improve with rest. Diminished or absent dorsalis pedis artery pulse is frequently observed. Diagnosis is primarily based on the presence of episodic limb pain, intermittent claudication, diminished or absent dorsalis pedis pulse, and migratory phlebitis.

First-line treatment involves pharmacotherapy, including dextran-40 (to improve microcirculation), vasodilators, antibiotics (if secondary infection is present), and corticosteroids. If significant pain persists, various analgesics may be employed. We hope this information is helpful to you.
