Clostridium tetani is an anaerobic bacterium commonly found in external environments such as soil and water. When skin and muscle tissues are injured—especially in deep wounds—this bacterium may cause infection. As an anaerobe, C. tetani thrives in low-oxygen environments; thus, deep wounds are particularly susceptible to infection. Following infection with C. tetani, tetanus vaccination is required.

In most patients, local skin wounds develop; if such wounds are not properly managed, they may become infected with *Clostridium tetani*, the bacterium that causes tetanus. So, what exactly is *Clostridium tetani*?

What Is *Clostridium tetani*?

Clostridium tetani is an anaerobic bacterium commonly found in soil, water, and other external environments. When skin or muscle tissue sustains an injury—especially a deep wound—it becomes susceptible to infection by C. tetani. As an anaerobe, this bacterium thrives in low-oxygen environments; thus, deep wounds pose a particularly high risk for tetanus infection. Following infection, tetanus vaccination (e.g., tetanus toxoid) is required.

After infection with C. tetani, patients initially experience prodromal symptoms, including fatigue, dizziness, headache, trismus (jaw muscle stiffness and soreness), irritability, and yawning. These prodromal symptoms typically last 12–24 hours, followed by characteristic severe muscle spasms affecting the masseter muscles, facial muscles, neck, back and abdominal muscles, quadriceps, diaphragm, and intercostal muscles. Patients begin experiencing difficulty chewing, restricted mouth opening (trismus), and lockjaw. Paroxysmal facial muscle spasms produce a distinctive “risus sardonicus” (sardonic smile). In severe cases, respiratory distress or apnea may occur, potentially threatening life.
