Eye size is primarily determined by genetics. In patients with monolids, the eyes often appear smaller. To achieve larger-looking eyes, comprehensive “enlarged-eye” surgery may be considered—such as incisional double eyelid surgery, levator palpebrae superioris muscle correction, or medial canthoplasty (inner canthal opening). These procedures can alter eye shape to varying degrees, increasing scleral and corneal exposure and thereby creating the appearance of larger eyes.
