Bilateral epididymal cysts predominantly occur in individuals aged 20–40 years. The most common location is the head of the epididymis, whereas cysts rarely develop in the body or tail. Clinically, patients often present with testicular discomfort; color Doppler ultrasound performed at a reputable hospital frequently reveals bilateral epididymal cysts. The etiology of bilateral epididymal cysts is complex: primary cysts may arise during fetal development, primarily associated with maternal exposure to diethylstilbestrol (DES), an estrogenic compound. In adults, epididymal cysts are mainly attributable to vas deferens obstruction, leading to impaired sperm transport and subsequent formation of spermatoceles (sperm-filled cysts). Small cysts typically do not impair fertility; however, large spermatoceles may compromise sperm quality and adversely affect fertility, necessitating timely surgical intervention.
