Darkening of the glans penis is generally a normal physiological phenomenon, commonly occurring after puberty due to gradually rising testosterone levels, which induce melanin deposition in the genital area—resulting in darker pigmentation. This typically does not require excessive concern. However, glans darkening may also indicate an underlying pathological condition—for instance, balanoposthitis (inflammation of the glans and foreskin). Recurrent bacterial infection in such cases can cause the glans surface to appear grayish or brownish and may be accompanied by pruritus (itching). Under a physician’s guidance, topical application of Mupirocin ointment (Bactroban) or erythromycin ointment may be used for treatment; once inflammation is controlled, circumcision may be considered.
