Recently, I’ve experienced unusual pain in my testicles; however, the pain is typically absent when I touch them manually. I’d like to ask: What could be causing this dull, persistent testicular pain—even though squeezing them doesn’t hurt?

This condition is commonly caused by chronic prostatitis, leading to pain in the epididymis and testes. In patients with chronic prostatitis, pathogenic microorganisms can infect the semen. These pathogens may trigger inflammation of the epididymis along the vas deferens, resulting in dull, persistent pain in the epididymis and testes. Patients may occasionally experience such dull pain in either the left or right testicle; however, palpation typically reveals no tenderness. Effective antibiotics—such as fosfomycin trometamol powder or levofloxacin tablets—may be selected, and in most male patients, testicular pain symptoms show marked improvement.
