Gagging while brushing teeth may result from stimulation of the pharyngeal mucosa—particularly the mucosa at the base and posterior part of the tongue—triggering a neural reflex that leads to coughing, nausea, or vomiting. Nausea during toothbrushing may also indicate gastroesophageal reflux disease (GERD), which commonly presents with symptoms such as nausea, acid reflux, and belching—especially after meals.
