Gastric bleeding is often painless because blood is alkaline and can neutralize gastric acid. In cases of massive upper gastrointestinal bleeding, symptoms include hematemesis (vomiting blood) and melena (black, tarry stools), and shock may develop. A common underlying cause is a bleeding gastric ulcer. Initially, the patient should remain in bed and avoid strenuous activity to prevent worsening of the condition. Once the patient’s condition has stabilized, an upper gastrointestinal endoscopy (gastroscopy) is required for definitive diagnosis and management.
