If herpes lesions are painless and non-itchy and appear on the genitalia, genital herpes—a sexually transmitted infection caused by the herpes simplex virus—should be considered. If the herpes lesions first appear behind the knees and then progressively spread toward the head and limbs in a centripetal distribution, varicella (chickenpox) caused by varicella-zoster virus infection should be suspected. For painless, non-itchy herpes lesions, it is recommended to consult a dermatologist to determine the underlying cause and receive appropriate treatment.
