Pain and swelling behind the male testicles are usually caused by varicocele. In most cases, varicocele is highly likely; however, part of epididymitis may also cause pain and discomfort behind the testicle, occasionally radiating to the abdomen. Severe varicoceles may require surgical treatment. Another possible cause is testicular torsion, which typically occurs at night or during rest. It is mainly caused by twisting of the spermatic cord, leading to testicular ischemia.
