Looking older after medial epicanthoplasty may occur when the patient's epicanthal fold is not particularly severe. If the epicanthal fold is mild, undergoing an eye-widening surgery can result in an unnatural eye shape—for example, an imbalanced ratio between the white and black parts of the eye—leading to a harsh, unattractive, or prematurely aged appearance.
