The most prominent feature of cerebral edema is increased intracranial pressure, which directly manifests as severe headache, vomiting (often projectile), papilledema, followed by impaired consciousness and changes in bilateral pupils. If intracranial pressure is not promptly controlled, leading to brain herniation, the bilateral pupils become unequal in size and irregular in shape.
