Whether an enlarged heart can return to normal depends on multiple factors. Generally, if the enlargement is caused by reversible factors in the short term—for example, acute myocarditis—then with timely and effective treatment, the heart may return to its normal size. However, if the enlargement results from long-term chronic conditions such as hypertensive heart disease or dilated cardiomyopathy, returning to normal is much more difficult.

Poorly controlled long-term high blood pressure gradually leads to structural changes in the heart. In such cases, even if blood pressure is well-controlled later, the extent of recovery of the heart is limited. For certain cardiomyopathies where the heart muscle itself is damaged, the possibility of full recovery is even lower.
