I've been having chest pain recently and suspect I might have myocarditis. My husband advised me to go to the hospital for a cardiac enzyme test. I'd like to know, can cardiac enzymes accurately detect myocarditis?

Myocardial enzyme testing has certain reference value in diagnosing myocarditis, but it cannot accurately diagnose myocarditis based solely on myocardial enzyme levels. Myocardial enzymes are a collective term for various enzymes present in the heart muscle, including aspartate aminotransferase, lactate dehydrogenase, creatine kinase, and others. When myocardial injury or necrosis occurs, these enzymes are released into the bloodstream, leading to elevated serum levels of myocardial enzymes. Therefore, myocardial enzyme testing can be used to assess the extent of myocardial damage. However, to confirm a diagnosis of myocarditis, clinicians must comprehensively evaluate the patient's symptoms, physical signs, electrocardiogram, echocardiography, and perform pathological examinations such as endomyocardial biopsy. Myocardial enzyme testing is only one component of auxiliary evaluations and cannot serve as the sole criterion for diagnosing myocarditis.
