Unilateral testicular pain with a sensation of heaviness and swelling may be caused by various factors. Testicular torsion is one possible cause, commonly occurring after vigorous physical activity or due to external forces—such as pressure during sexual intercourse or while sleeping—that lead to twisting of the testicle, resulting in pain and swelling. Varicocele, a vascular condition, may also cause unilateral testicular discomfort characterized by a feeling of heaviness and pain in the scrotum, which may radiate to the ipsilateral groin or lower abdomen. Additionally, orchitis or epididymitis caused by pathogen infection can lead to such symptoms, often accompanied by redness of the scrotal skin and testicular enlargement. Other potential causes include urinary tract infections and kidney stones.
