Frequent palpitations are not necessarily indicative of myocardial ischemia. Although myocardial ischemia may cause palpitations during an episode, the causes of palpitations are diverse, including both physiological and pathological factors. Physiological causes include mental stress, vigorous exercise, smoking, and consumption of strong tea or coffee. Pathological causes are more complex and may include left ventricular hypertrophy due to hypertension, heart failure, arrhythmia, anemia, hyperthyroidism, and autonomic dysfunction.
