My toenail has separated, and the color remains grayish. After examination, it was diagnosed as a fungal nail infection (onychomycosis). I would like to ask the doctor, what is the treatment for onychomycosis and how can it be cured?

Dr. Wang Xiaoyan
Onychomycosis, medically termed tinea unguium, is a common dermatological condition caused by various fungi invading the nails. Its symptoms include nail discoloration, thickening, brittleness, and sometimes subungual debris accumulation or nail separation. Treatments for onychomycosis include the following: 1. **Topical medications**: Such as creams, solutions, or nail lacquers containing antifungal ingredients. Consistent application over a prolonged period is required, typically lasting 6 months to 1 year. 2. **Oral antifungal drugs**: Such as itraconazole and terbinafine. These require a physician's prescription, and potential drug interactions and side effects should be carefully monitored. 3. **Nail avulsion**: Although nail removal can eliminate the affected nail, it is associated with significant trauma and a high recurrence rate. Therefore, it is usually reserved for the treatment of isolated onychomycosis without concurrent athlete's foot. Sufficient antifungal therapy is also required after nail avulsion to prevent recurrence. 4. **Laser therapy**: Utilizes the thermal effects of lasers to destroy or kill fungi, thereby treating onychomycosis. Multiple treatment sessions are usually necessary, but recurrence is less common. 5. **Foot hygiene**: Soak feet daily in warm water and wash them with a mild cleanser, especially between the toes. After washing, gently dry the feet thoroughly with a clean towel, paying particular attention to the areas between the toes.
