Scrotal enlargement and the presence of a mass may be caused by various conditions, including but not limited to the following:

1. Testicular tumor: It may be malignant, such as testicular cancer, or benign, such as varicocele. In this case, the mass usually increases gradually, sometimes accompanied by pain or discomfort.

2. Testicular torsion: This is an emergency condition caused by twisting of the blood vessels of the testicle, leading to blocked blood flow, severe pain, and rapid swelling.

3. Spermatic cord cyst or epididymal cyst: These are fluid-filled sacs resulting from fluid accumulation. They are generally painless, but a mass may be felt.

4. Orchitis or epididymitis: Caused by infection, it is usually accompanied by redness, swelling, pain, and fever.

5. Hydrocele: A painless fluid accumulation, commonly seen in children but can also occur in adults.

If concerned, it is recommended to seek medical attention promptly.
