What is the most effective medication for gray nail?
Disease description:
Female, 45 years old. My recent case of onychomycosis (gray nail) has been worsening. What medication is most effective for treating onychomycosis?

Onychomycosis, medically known as tinea unguium, is a fungal infection that typically requires long-term treatment. The most effective medications for treating onychomycosis mainly include the following categories:
1. Topical medications: For mild cases where the fungal infection is limited to the nail surface or for patients who cannot tolerate oral medications, compound salicylic acid solution can be applied until new nails grow completely. Alternatively, urea ointment can be used under occlusive dressing to soften and remove the infected nails, followed by topical application of amorolfine hydrochloride cream, econazole nitrate solution, or other similar medications.
2. Oral medications: If the entire nail turns gray and becomes thin and brittle, indicating deep fungal infection involving the nail bed and surrounding tissues, topical medications may not adequately penetrate to treat the infection. In such cases, treatment with oral antifungal medications such as itraconazole capsules or fluconazole capsules should be taken under medical supervision.
3. Combination of topical and oral therapy: For severe cases of onychomycosis, combining topical treatments with oral medications can enhance treatment effectiveness.
