The main causes of acne on the scrotum include: 1. Poor hygiene: Inadequate personal hygiene habits, such as infrequent bathing or not changing underwear regularly, can irritate the scrotal skin and lead to acne formation. 2. Folliculitis: Inadequate cleanliness or excessive local sweating can easily lead to bacterial infection, causing folliculitis. This presents as acne-like papules on the scrotum and may be accompanied by redness and pain. 3. Scrotal eczema: Moist conditions or allergens affecting the scrotum may trigger scrotal eczema, presenting as papules or vesicular papules on the scrotum, accompanied by itching and swelling. 4. Ectopic sebaceous glands: Related to increased androgen secretion during puberty or long-term irritation of the local skin, these may form small papules the size of a pinhead on the scrotum. 5. Genital warts: Caused by infection with human papillomavirus (HPV), these may appear on the scrotum as single or clustered small pale red bumps. As they grow larger, they may present as cauliflower-like or papillary white skin growths.
