Drug metabolism and excretion times can be influenced by individual body differences, drug dosage, liver and kidney function, and other factors.

Tramadol is a centrally acting analgesic, and its primary metabolite is O-desmethyltramadol, which is active. The half-lives of tramadol and its metabolites are approximately 4-6 hours and 7-9 hours, respectively. Within 24 hours, about 80% of tramadol is excreted in the urine in its original form and as metabolites. Complete clearance of tramadol from the body takes longer for urine tests to return to normal. Patients with faster metabolic rates may fully excrete tramadol within 3 days, while those with slower metabolism may take up to 5 days for complete excretion.

However, generally speaking, in healthy adults, tramadol may no longer be detectable in the urine within 2-4 days after the last dose. The sensitivity and specificity of urine drug screening can also affect test results. In some special cases, even after most of the drug has been metabolized, false-positive or false-negative results may still occur.

Urine test results can also be influenced by diet and drug interactions; medications or foods that may interfere with urine test results should be avoided prior to testing.
