Elevated bilirubin levels in pregnant women, especially increased direct bilirubin, may indicate intrahepatic cholestasis of pregnancy or other liver diseases. Intrahepatic cholestasis of pregnancy is a specific complication of pregnancy, mainly characterized by elevated serum total bile acid and bilirubin levels, accompanied by pruritus. The management principles are as follows:

1. Dietary adjustment: Pregnant women are advised to follow a light and balanced diet, ensuring adequate caloric intake, protein, and vitamins. High-quality, high-protein foods such as fish, eggs, and milk can be consumed. Additionally, sufficient water intake is recommended to replenish bodily fluids, improve circulation, and promote metabolism.

2. Lifestyle management: Pregnant women should maintain healthy lifestyle habits, avoid excessive fatigue and staying up late, and ensure adequate rest.

3. Pharmacological treatment: Ursodeoxycholic acid is the first-line treatment for intrahepatic cholestasis of pregnancy. It effectively reduces serum bile acid levels and improves maternal and fetal outcomes. Antihistamines or short-term corticosteroids may be added for severe pruritus.

4. Surgical treatment: If a pregnant woman is diagnosed with elevated bilirubin caused by gallstones and conservative treatment is ineffective or serious complications arise, surgical intervention such as laparoscopic cholecystectomy may be required.
