I received a single-chamber right ventricular pacemaker 21 years ago and have since undergone two additional replacements, all of which were single-chamber pacemakers. I am now experiencing diastolic heart failure, which has been progressively worsening. How should chronic heart failure induced by single-chamber pacing be managed?
Heart failure has developed, requiring long-term medication to improve cardiac function, enhance myocardial contractility, promote diuresis, and nourish the myocardium. Implantation of a single-chamber pacemaker serves only to maintain normal cardiac rhythm. It is recommended that the patient consult a cardiologist for comprehensive evaluation, including tests such as complete blood count, electrocardiogram (ECG), 24-hour Holter monitoring, echocardiography, cardiac enzymes, and brain natriuretic peptide (BNP), to establish a definitive diagnosis and initiate appropriate treatment.
