Heart sounds may also change: the first heart sound at the apex may be diminished or split, and the presence of pericardial friction rub indicates pericarditis. Symptoms of viral heart disease include heart rate variability; heart rate increases proportionally with body temperature, whereas abnormally slow heart rate may indicate myocarditis. Heart sounds may also change—first heart sounds at the apex may be diminished or split, and the presence of pericardial friction rub suggests pericarditis. Systolic or diastolic murmurs may be heard at the cardiac apex. Additional manifestations may include fever, anemia, and ventricular enlargement. Relative mitral stenosis secondary to left ventricular enlargement may produce a murmur graded below Grade III, which typically resolves as myocarditis improves.

Atrial and ventricular premature beats are the most common arrhythmias, followed by atrioventricular (AV) conduction block. Atrial fibrillation and sick sinus syndrome may also occur. Arrhythmias constitute one of the leading causes of sudden cardiac death. In severe, diffuse myocarditis, acute heart failure may develop due to myocardial pump failure—simultaneous failure of both left and right ventricles results in low cardiac output. Thus, in addition to typical signs of heart failure, patients are prone to develop cardiogenic shock.

Patients with viral myocarditis should prioritize rest: strict bed rest is recommended during the acute phase, with activity strictly limited. During recovery, excessive fatigue must be avoided, and vigorous physical exercise is contraindicated. Patients should avoid direct sun exposure, maintain appropriate indoor temperature control, and take precautions against colds and upper respiratory infections. Close monitoring of clinical status is essential; prompt emergency intervention is required if life-threatening symptoms such as dyspnea, pallor, or unpalpable pulse emerge.
