The exact cause of lentigo simplex remains unclear. Unlike freckles, which are primarily influenced by genetic and hereditary factors, the pathogenesis of lentigo simplex is not well understood, and there is no clear evidence of heritability. Its development has no established association with trauma or infections caused by bacteria, viruses, or fungi.

Some women develop freckle-like nevi on their faces, which persist despite numerous treatment attempts. So, are freckle-like nevi truly impossible to remove?

Freckle-like nevi are a common pigmented dermatological condition and may occasionally serve as a clinical feature of certain inherited syndromes. They can appear anywhere on the skin but are most frequently observed at cutaneous–mucosal junctions. Clinically, they typically present as discrete, brown macules measuring 1–2 mm in diameter. Lesions often occur unilaterally and may gradually enlarge with age. Unlike true freckles (ephelides), freckle-like nevi are darker and more widely scattered; their appearance is unaffected by sun exposure—neither their number nor color intensifies following tanning—and they are generally asymptomatic. Treatment options include Q-switched laser therapy. Although results are less predictable than those achieved for freckles, most freckle-like nevi can be significantly lightened or completely eradicated.

The precise etiology of freckle-like nevi remains unclear. In contrast to freckles—which arise primarily from genetic and hereditary factors—the pathogenesis of freckle-like nevi is not well defined. Notably, no clear familial or hereditary pattern has been identified. Furthermore, their development shows no established association with trauma or infections caused by bacteria, viruses, or fungi.
